Job Summary
Geisinger is seeking an experienced Nurse Practitioner or Physician Assistant to join our Skilled Nursing Facility team for an open position to work between the Milton and Watsontown facilities.
Job Duties
Nursing home/skilled nursing facility based Advanced Practitioner (NP or PA-C) works in conjunction with CMSL clinical staff, applicable insurance case management staff, and skilled nursing facilities to ensure patients are appropriately managed in the sub-acute and/or long-term care setting. The SNF advanced practitioner (AP) provides direct (hands-on) care in nursing homes/skilled facilities with a goal to improve quality and prevent unnecessary re-admissions to the acute care setting.
MAJOR DUTIES AND RESPONSIBILITIES:
1. SNF AP is a field-based patient direct care position providing medical services for one or two facilities. Workload is 90% clinical, 10% administrative. The SNF AP works under the direction and supervision of the supervising/collaborating CMSL physician.
*2. Performs comprehensive SNF assessment to include psychosocial factors (when
applicable) identifying the anticipated short-term and long-term (ongoing) needs of the patient.
*3. Responsible for the treatment of patients in the nursing home and skilled nursing facility settings.
4. Educates facility staff, patients and/or families about preventive care, medical issues, and use of prescribed medical treatments and/or medications.
5. Maintains open lines of communication with an emphasis on supportive relationships with assigned nursing homes/skilled nursing facilities,
*6. Maintains legible and accurate medical records which meet state and federal regulations and maintains patient confidentiality.
*7. Must be able to take weeknight call on all assigned patients and participates in weekend call rotations.
8. Works collaboratively with appropriate insurance management staff, including:
a. Communicating clinical issues with potential for high risk complications and barriers to positive outcomes and discharge (as appropriate);
b. Working collaboratively with insurance case management staff, CMSL, facility and the patient and family to establish a plan of care that identifies and continuously reassesses appropriate levels of care;
c. Identifying patients that would benefit from an alternative level of care;
d. Establishing short and long term goals that meet the patient's needs;
e. Try to prevent ED visits;
*9. Identifies and refers patients to appropriate disease and case management programs;
*10. Identifies the need for hospice care as appropriate in the terminal period of chronic diseases. Refers appropriate patients to a case manager for hospice program election or to be designated as a Potential Hospice candidate and monitored for decline.
*11. Will be responsible for meeting ongoing continuing medical education requirements and provide copy of documentation for internal and external continuing medical education attended to the office of credentialing.
12. All other duties as assigned.

EDUCATION AND/OR EXPERIENCE:
Must have certification of completion from an approved program for Nurse Practitioners and have or be eligible for current license in the Commonwealth of Pennsylvania. Must have BLS and have or be eligible for ACLS.
Prior experience in nursing home/skilled nursing facility billing (codes, understanding of medical necessity).
Prior advanced practitioner experience in a long-term care setting and/or administrative experience in a skilled nursing facility or long-term care facility is preferred.
WORKING CONDITIONS/PHYSICAL DEMANDS:
Ability and willingness to travel.
Ability and willingness to take call after hours and participate in weekend call rotation.
Work is typically performed in a skilled nursing facility.
