The Whole Health Coach works with individuals and groups throughout the facility in a client-centered process to facilitate and empower the client to develop and achieve self-determined goals related to health and wellness. The Whole Health Coach requires broad knowledge, skills, and training and exercises independent judgement. The Whole Health Coach is expected to be involved to a greater degree in participating in the development of patient-centered care plans. Duties include but are not limited to: Promotes therapeutic alliances between patients and their personal support persons by using health education interventions, health coaching, motivational interviewing, and self- care strategies. Is responsible for the facilitation of care coordination and integration of care of the whole health model by leading mind-body skills groups, workshops, and individual sessions for patients and staff to promote physical and emotional wellness, decrease burn-out, and improve morale. Provides Health and Wellness Coaching and Mind-Body Skills Training though various VHA approved methods that meets the patient need and preference, including telephonic, tele-video, electronic, in-person individual and group settings. Documents care, coaching, and skills training provided, observations and responses on appropriate tools or forms, observing changes in patient's condition, personal health preferences and values, and readiness for change (e.g. behavior shifts, motivation, personalized health plan, etc.) Utilizes motivational interviewing and health coaching techniques and processes, learned through training, to assist veterans in developing a realistic plan to accomplish their goals. Participates in the interdisciplinary treatment team meeting process, including sharing observations and patient's responses to interventions, coaching and skills training. Oversees Complementary Integrative Health to include Yoga, Tai Chi, Qigong and meditation classes. Conducts outreach activities to inform Veterans, and family members, of available programs. They represent the VA Health Care System, and Integrated Health Program, at community programs and outreach events. Responsible for managing their clinic workload and ensuring their documentation accurately captures their productivity. Serves as a preceptor in orienting, educating and training less experienced WH coaches.
