The position will be assigned to the Home Based Primary Care program and practices as a Case Manager (RN) within the VA Boise Medical Center. The role of Case Manager is responsible for managing care across the health care continuum for a select group of clients. He/she provides professional assessment, coordination, telephone triage and planning of multiple health care services and acts on behalf of the veteran to ensure necessary clinical services are received and progress is being made. The duties and responsibilities of the position include but are not limited to the following: Is responsible to facilitate communication and collaboration between all members of the healthcare team involving the patient and family in the decision-making process; to minimize fragmentation of healthcare delivery; to optimize quality of patient and family outcomes in a cost-effective manner. Works effectively with all levels of staff, to include collaborative efforts with other health professionals providing ongoing case management services. Responsible and accountable for the initial and continuous assessment for a culturally diverse veteran patient population in the home setting. Provides teaching to the patient and caregiver in ways of managing and delivering nursing care in the home. Monitors patient's condition and the care required in the home and collects blood and other specimens required for monitoring the effectiveness of interventions and/or disease progression. Provides needed assistance and consultation for home care referrals. Possesses the knowledge and skills to effectively and collaboratively apply all aspects of the nursing process within the area of assignment. Demonstrates knowledge and skills necessary to assess the care appropriate to the age related needs of patients served in the home setting. Understands changes associated with aging and principles of growth and development relevant to the adult and geriatric population.
