The Registered Nurse Orthopedic (Care Coordinator) in the Orthopedic Surgery Service provides consultative orthopedic surgery care service. The Orthopedic Surgery service includes outpatient and inpatient care and specializes in the delivery of comprehensive care of patients requiring Orthopedic Surgery interventions and pre- and post-surgical treatment. The RN Care Coordinator uses the nursing process and evidence-based practice to collaborate with the PACT team members (Patient, MSCC Provider, Medical Support Assistant (MSA), and LPN and larger Team (family/caregiver, internal and community-based services involved in providing care to the patient) in developing the patient-driven holistic care plans. He/She is responsible for collaborating with services internal and external to the VA to facilitate transition of care that effectively meet the patients' needs. This includes but is not limited to chart reviews, telephone calls to patients for pre-visit preparation or post visit follow up, participation in team huddles, and collaborative management of high risk patients. The RN Care Coordinator provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals. The scope of care provided by the nursing personnel includes: Pre-surgical care, access management, and follow up post-op Assist in operating room as a scrub staff Monitoring of patients and performing follow-up calls Nursing interventions Schedules for surgery with OR scheduler Enters and monitors data in the Surgery package for surgery wait times Completes open consults within 7 days or less Serves as a resource person for the inpatient nursing staff on orthopedic issues/concerns Monitors and audits appointments to ensure access is within 30 days Follows all patients that are sent out on a fee base (Non-VA facilities) until they return back to the VA Rounds with Residents and/or Providers on orthopedic inpatients as needed to ensure all issues are addressed Work Schedule: 8 hour Day shift that will start between 7:30am - 8:00am Telework: A Adhoc Telework Only as Needed Virtual: This is not a virtual position.
