Note: Additional selection will be made for position located at the St Marys CBOC. Whole Health Coaches provide care to Veterans seeking self-directed, lasting changes aligned with their values. They provide services that promote health and wellness, enhance well-being, improve health related outcomes, reduce likelihood of inpatient admission, and improve quality of life. Coaches support Veterans in mobilizing internal strengths and external resources and developing self-management strategies for making sustainable, healthy lifestyle behavior changes and managing chronic disease more effectively. Coaches work with Veterans on identifying the Veterans' motivation for change, determining goals for their work together, and assessing and supporting existing treatment plans to reach these goals. They provide information on the relationship between lifestyle and health outcomes, exploring behaviors that increase the risk of developing or worsening chronic disease and those that reduce the likelihood of chronic illness. A Whole Health Coach performs the following duties: Contacts eligible participants for enrollment and explain the health coaching process. Provides health coaching for Veterans to reduce or eliminate high-risk behaviors while increasing healthy ones. Provides follow-up care through telephone, VA Video Connect or secure messaging between face-to-face appointments. Schedules ongoing sessions to assess progress and assist in navigating obstacles or challenges. Encourages and assists Veterans with the adoption of habits that are conducive to a higher quality of life and in alignment with the Veteran's goals. Identifies habits and behaviors that are harmful to quality of life and related to chronic illness. Schedules ongoing sessions to assess progress and assist in navigating challenges. Guides Veterans to learn about available support tools in the VA and the community that assist in attainment of goals. Collaborates with the Whole Health team in planning and implementing services and coordinating outreach activities. Note: Additional selection will be made for position located at the St Marys CBOC.
