The Advanced Practice Registered Nurse (APRN) is assigned to the Urgent Care Service of the VA Roseburg Medical Center, Roseburg, OR. The APRN functions as an independent provider and oversees care provided to patients in the Urgent Care Clinic. This includes identifying the patient's biopsychosocial concerns, defining the management of these problems, and determining the need for treatment, consultation, or referral. The APRN consults/collaborates with healthcare clinicians and other team members. The APRN attends medical staff meetings, team meetings, and provides assistance in the implementation and application of all programs as required. The APRN provides leadership in improving patient care, maintaining standards of care, and teaching personnel in a variety of settings. The needs of patients are identified and appropriate referrals are initiated for other programs and services. The incumbent is responsible for leading the multidisciplinary team in coordinating and supporting care activities from the point of diagnosis and across the continuum of care through disposition for patients receiving services at VARHS. The APRN provides leadership that facilitates the delivery of quality coordinated care through the application of the nursing process, medical model, and using patient-centered principles of care.
