The VA Greater Los Angeles Healthcare (VAGLAHS) is seeking a Advanced Practice Registered Nurse (APRN) to serve in our Surgical Service area. APRN is a registered nurse with national certification & master's degree in nursing or higher. Responsible for assessing, diagnosing, planning. implementing, & evaluating direct & indirect health & quality of care for patients under his/her care, can be provided in a primary, secondary, or tertiary care setting. Will be based in our San Luis Obispo clinic Typical duties include but are not limited to: Function as expert in preop care for the surgical services of ENT, dental, plastic, podiatry, gynecology, orthopedics, urology, general, vascular and neurosurgery and procedural services including (but not limited to) GI, cardiology, and radiology. Perform thorough medical risk stratification, which includes a detailed history and physical, medical chart review of pertinent medical records, laboratory review, review of radiologic studies, and evaluation/interpretation of other diagnostic studies, including EKGs, cardiac stress tests and pulmonary function tests. Evaluation may include determining whether the patient needs additional testing or evaluation by a subspecialist and arranging those tests and consults. Follow up on any pending labs, studies, or consults that may affect the patient's perioperative course Evaluate and anticipate medical/surgical/psychiatric emergencies and initiate/lead others to implement appropriate and timely action preop to avoid surgery cancellation/delay. Establish patient care priorities based on patient needs that ensure surgery/anesthesia can be delivered in a safe, efficient and cost effective and timely manner Coordinate patient care with other specialty clinics and departments (such as cardiology and pulmonary) and consult them on an as-needed basis. Offers anesthesia and surgical colleagues' perioperative recommendations, such as for perioperative beta-blockade, stress-dose steroids, and endocarditis prophylaxis. Follows clinic's standard operating procedures, such as the use of heparin "bridging" for patients on chronic warfarin. Provide patient education regarding perioperative medication management, including antihypertensive medications, diabetic agents and oral anticoagulants. Evaluate, promote and advocates for and strive to protect the health, safety and rights of patients throughout the preop process Make sustained contribution to healthcare inside and outside of the medical center as evidenced by professional presentations, academic teaching (precepted anesthesia, dental, medical and nursing students). Advocate for patients and go beyond the duties that are required to ensure that the patient gets all necessary care, whether it is follow-up with primary care provider or specialty referral. Lead and organize delivery of preoperative care. Guide preoperative staff in the application of critical thinking and process improvement to improve preop care. Initiate and lead the interdisciplinary group in critical thinking processes that lead to decision that positively impact and increase efficiency in the preoperative process.
