The RN is responsible for the provision of nursing care for patients in Mental Health (Nursing) Service, Central Alabama Veterans Health Care System (CAVHCS) according to applicable VHA Directives and ANCC standards of nursing care and practice. The Registered Nurse will report to the Nurse Manager or Associate Chief Nurse. Duties include but not limited to: Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction. Considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences. Implementation of approved policies and procedures. Demonstrates proficiency in practice based on conscious, deliberate planning Self-directed in goal setting for managing complex patient situations. Conducts assessments on all patients presenting for care. Functions as an integral member of the treatment team and assumes responsibility for completing assessments for patients presenting. Safely and effectively uses patient care equipment. Recognizes overt abnormal signs and symptoms and/or changes in patients' condition and responds appropriately. Completes initial assessment of patient condition and response to treatment and initiates follow-up as needed. Develops and implements a plan to meet learning needs, instructing patients and families in basic health teaching and discharge planning. Documentation is consistent, accurate, concise, and timely. Revises plan of care according to evaluations of patient's responses and outcomes. Uses the Computerized Patient Record System (CPRS) and the Bar-Coded Medication Administration (BCMA) software per policy. Practice follows the ANA Code of Ethics for Nurses. Treats all patients and families with dignity and respect. Demonstrates compliance with infection control practices, such as hand washing and wearing of appropriate personal protective equipment. Complies with safety rules and regulations for safe job performance and fire protection.
