Duties of this position include but are not limited to: The Nursing Assistant performs and records basic and standardized nursing care, including: Observes and performs personal nursing care duties and related support duties, including feeding, bathing, lifting, turning and repositioning, oral care, nail care, and related personal hygiene duties. Takes vital signs, report results to the primary Registered Nurse (RN) and records results in the medical record. Reports changes in patient condition to the RN. Weighs patients, report results to RN and enters result into the medical record. Provide age related competent care. Collects and labels specimens (urine and stool only). Transports all specimen types to the laboratory. Maintains and stores medical supply items and equipment. Places clean linen in proper storage area. Collects and bags soiled linen for the laundry. Fills water pitchers and gives ice water to patients at beginning of shift and as needed. Distributes and collects food trays. Feeds patients or assists with meals as needed. Reports to the RN and documents input and output of fluids into the medical record. Participates in patient care activities off the unit, and off-site. Accompanies patients for care and assessment at other facilities. Transports patients for procedures/tests as required. Make pertinent observations during transportation of patients to and from appropriate areas. Ensure they are protected by clothing to keep them warm or cool as indicated. Assists RN with a range of treatment procedures such as sitz bath, set up of equipment for oxygen therapy and reinforcing with patients to use incentive spirometer. Ambulates patients as directed by the RN. Assists RN with inventory of patient belongings and documents in the medical record. Takes emergency measures, calls for Nurse and/or Physician and assists with resuscitation procedures in medical emergencies within scope of practice. Recognize and report emergency situations and to initiate appropriate actions within scope of practice. Set up telemetry monitoring as directed by RN if training and competency documented. Maintain cleanliness of telemetry monitoring equipment. Performs/assists with post-mortem care. Observes and reports breakage and loss of equipment. Enters electronic work orders and notifies RN and supervisor. Performs other duties as assigned, normally of a related nature and within scope of practice. Does one-to-one and constant observation. Does every 15 and every 30-minute checks, when assigned. Is knowledgeable about interventions in the Prevention & Maintenance of Disruptive Behavior. Basic medical terminology and grammar for documentation of patient care in the medical record. Understand and follow the goals established for patients as communicated by the RN; communicating with nurses; patients, families and supporting medical staff. Actively promote and provide infection control, and follow universal precautions, hand hygiene and provide environmental safety. Knowledge of the EKG machine and associated minor maintenance and cleaning. Knowledge of, and skills sufficient to obtain 12-lead EKG's, and in telemetry units, knowledge to place EKG leads. Maintains a clean environment in accordance with antiseptic standards. Maintains a neat environment that is quiet and promotes a calm, supportive relationship with patients/families, and/or caregivers. Observes fire regulations and eliminates hazards to avoid injury or accidents. Ability to communicate effectively both orally and in writing when rendering services to customers (patients) from diversified backgrounds in order to obtain and receive sensitive information in a professional manner. Knowledge of documentation specific to NAs. Knowledge of legal issues related to NA practice. Participates in unit and interdisciplinary meetings as appropriate.
