This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Learn more The Transfer Coordinator is assigned to Health Care and Business Operations (HCBO). The incumbent is primarily stationed at the Perry Point Medical Center and has responsibilities at all hospital-based sites throughout the VAMHCS. The Transfer Coordinator is responsible to facilitate acute care transfers and admissions within and outside the VAMHCS and VISN 5. The incumbent is responsible for ensuring the appropriate communication of patient care and customer service needs between medical centers. The Transfer Coordinator coordinates both the exchange of clinical information about the veteran patient and the logistics of moving the patient safely between medical facilities for inpatient transfers and admissions. The Transfer Coordinator monitors and resolves any safety or quality of care issues arising from the transfer or referral of patients. The incumbent collaborates with physicians, case managers, nursing staff, utilization review staff, and MAS staff specifically the bed coordinators and other members of the health care team to assure appropriate and safe transfer of patients.
