The Registered Nurse Office of Community Care is a professional member of the health care team providing care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process in the community-based programs at the Gulf Coast Veterans Health Care System. The Registered Nurse Office of Community Care is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Duties include but not limited to: *Assess, plan, implement and evaluate care based on age-specific components *Assumes responsibility for the coordination of care focused on Veteran transition through the continuum of care, patient and family education *Administers medications and procedures per established policies and guidelines *Influences care outcomes by collaborating with members of the interdisciplinary team *Knowledge and active participation in unit/program level quality improvement processes and initiatives as well as customer service programs Work Schedule: Monday- Friday (8:00am- 4:30pm) Financial Disclosure Report: Not required Recruitment/Relocation Incentives: Not Authorized Relocation Expenses: Not Authorized
