The PACT Care Coordinator is a Registered Nurse (RN) who will serve as a champion for patients and coordinates local implementation, training, communication, outreach and sustainability of PACT concepts and the principles of the PACT team. The nurse works in conjunction with the PACT team to ensure comprehensive, continuity, and continuation of care.The RN is responsible for coordinating and participating in huddles to ensure continuity of care within the PACT teams.
