PRN

• Timely, accurate, and thorough completion of medical documentation
• Ability to give immunizations
• Appropriate documentation of patient information on intake (including identified standards for documentation â“ i.e. mammogram, colonoscopy, smoking, etc)
• Ordering of pertinent medical supplies (including pharmaceuticals) and keeping inventory stocked
• Ability to utilize computer/computer software for pertinent patient documentation, providing documentation to patient as needed (i.e. patient plan).  This includes pertinent training as necessary from time to time
• Adhere to policies/procedures for patient medications and testing â“ refilling patient medications according to protocols (SMPG policies)
• Electronic communications with patients, when desired (in keeping with established policy standards)
• Review of patient information via huddles to comply with established PCMH measures, with appropriate follow-up and/or communication as necessary.
• Excellent communication (both written and verbal)
• Effectively coordinate patient care with pertinent referrals and/or follow-ups (within protocol/policy standards) and in keeping with PCMH established standards
• Meaningful use compliance to standards
• Timely management of patient needs/requests (including phone calls) in accordance both with policy and established PCMH standards
