This position is a M-F salaried position supporting the Pediatric Heart Institution at MemorialCare Miller Children's & Women's Hospital Long Beach.
The Cardiac Clinical Program Coordinator working in Pediatric Heart Center serves as a liaison between the patient/family and healthcare team for assessment, coordination of diagnostic work-ups, treatments and services, referrals for outside/contracted services, follow-up of study results, and documentation of the progress of their condition across the continuum of care. This position will focus on the coordination of the patient's care for the patient population of pediatric congenital heart disease, in collaboration with the entire healthcare team, over a prolonged period of time, in the pediatric setting with transition into an adult setting. This position will also function as a member of the multidisciplinary CORE team in the outpatient clinic setting to provide/ensure nursing assessments, contribute to the plan of care, coordinate team conferences and provide patient/family education.
3+ years experience with this specific patient population.
3+ years of experience in large, complex tertiary medical center with a focus in Cardiac Care preferred.
Excellent computer skills in a Microsoft Windows environment including Word, Excel, and PowerPoint.
Excellent written and verbal communication skills.
A Bachelor's degree in Nursing (BSN) is required; prefer MSN.
Current American Heart Association Basic Life Support (CPR & AED) and Pediatric Advanced Life Support (PALS) certifications are required pre-hire.
