Rockford Center is seeking a dynamic and talented Admissions Registered Nurse (RN).
Rockford Center is Delawareâ€™s first private psychiatric facility and remains a leader in providing behavioral health care services and continues its commitment to providing quality health care.Â Rockford Center, a 138-bed acute psychiatric hospital continues to provide a full continuum of behavioral health services to geriatrics, adults, adolescents, and children.
The Admissions RN will deliver quality nursing care to psychiatric adult, geriatric, adolescent and child patients.
Key Responsibilities include:
Assess patients through a systematic process of interacting with patient, the patientâ€™s significant others, and the appropriate health care providers during the admission of a patient.
Collect and analyze data regarding the patientâ€™s physical, psychosocial, environmental, self-care, educational, and anticipated discharge planning needs.
Reviews labs for patientsâ€™ alcohol or drug toxicity, identify signs of withdrawal, and assess other medical complications that will require additional care at Rockford Center.
Develop a comprehensive outline of care and specific goals and interventions to be included in treatment planning.
Complete nurse-to-nurse reports for incoming patients and review labs for admissions (when available).
Complete the nursing admission when available to include: database, treatment planning, flow sheet, admit note, medication reconciliations and admission orders.
